01:PROCEDURE

What kind of procedure are you having? Is it an invasive procedure?.From minimally invasive to surgical - it needs to be done by a doctor or plastic surgeon depending how invasive the procedure is. Even among medical practitioner, each of us has our limitation to what we can do and can't do, depending on our qualification and training.

Now that you know that the procedure is invasive and can only be done by doctors? Always make sure your procedure is using safe machine and drugs that is approved by Medical Device Authority (MDA) and National Pharmaceutical Regulatory Agency (NPRA). They are responsible for protecting the public health by assuring the safety, efficacy, and security of human and veterinary drugs, biological products, medical devices, our nation's food supply, cosmetics, and products that emit radiation

02: PRACTITIONER

For minimal invasive or invasive aesthetics beauty treatment, in Malaysia it can only be done by doctors with special qualification and license. The special license required is called LCP (Letter of Licensing & Credentialing) issued by Ministry of Health Malaysia.

LCP can be divided into category according to level of expertise and level of invasiveness limitation, of your doctor. Level 1 as the least invasive, while Level 3 being the most invasive.

Non Specialist

LCP Chapter 1 : General practitioner with special interest in aesthetics

They are medical officers who has underwent 3 years of aesthetic training.
They are allowed to perform only non-invasive and minimally invasive procedures up to dermis layer (skin layer).
They are registered doctors with Malaysian Medical Council (MMC)
They are LCP Chapter 1 holder by MOH

In the table on the right are procedures that can be done by LCP Chapter 1 Holder.

Specialist

LCP Chapter 2 :

Core : Dermatologist
Non Core : Other medical specialist who is allowed to do specific aesthetic procedures.

They have completed master in general medicine with subspecialization of dermatology.
They are allowed to perform only non-invasive, minimally invasive and invasive procedures up to below dermis layer (skin till fat layer).
They are registered doctors with Malaysian Medical Council (MMC)
They are a holder of LCP Chapter 2 by MOH
They are certified specialist registered under National Specialist Registry

PREMISE

Alhamdulillah for the blessed Ramadan. This month, in addition to handling various cases such as Scarless Facelift and other facial rejuvenation procedures, as well as reconstructive surgeries for trauma, tumors, and congenital malformations, I also embarked on a new venture by joining one of the leading aesthetic clinics in the country, Glojas as part of my current practise.

✨POST-BARIATRIC BREAST MAKEOVER✨

You voted in the poll to see the full results, and here they are. Firstly, I would like to thank my patient for willingly sharing her journey here. She really wanted to share it with her friends and peers but was afraid of being judged for undergoing cosmetic surgery, even though she is really happy and can now be comfortable again in her own body.

This patient is in her late 30s and had undergone a bariatric procedure, successfully losing a significant amount of weight. However, the weight loss took a toll on her breasts, causing them to become deflated and saggy, which resulted in her losing her confidence. After discussing it together with her husband, they decided to come see me for a breast makeover consultation and advice.

::: What Was the Problem? :::
* Breast sagging (Ptosis) Grade 3
* Significant loss of volume, especially at the upper pole
* Abnormally large nipple size (due to post-pregnancy rather than weight loss)
* Very thin skin with lots of laxity

::: What Was Done? :::
* Breast augmentation with silicone breast implants (Dual plane)
* Breast lift (anchor, a.k.a. wise pattern incision)
* Nipple reduction (without areolar reduction)

The surgery went uneventfully, as well as the post-surgical recovery. The before-and-after photo is six months post-surgery, with final results expected up to one year.

::: What to Expect at 6 Months? :::
* Implants will start to position themselves towards the final results.
* Breasts are softer and more natural to touch.
* The contour usually has good rounding and curves, especially at the side and bottom.
* Implants can still change gradually up to 1 year, so don’t stop your follow-up yet.

It brings me immense joy to witness the happiness of both the patient and her husband. Only then can I truly be happy for them.

This video amassed 2.7 million views on my TikTok, and I’m unsure about the reason.

It was part of my surgery, of a middle-aged woman’s transformation after losing 40kg through her weight loss journey post-bariatric surgery. Despite successfully shedding excess weight, she couldn’t control where the loss occurred.

The decline in breast volume post-weight loss is a common issue in my practice. Patients often seek breast lift, implants, fat transfer, or a combination thereof. Many women keep such experiences private due to societal stigma and judgment.

During clinic visits, questions arise about the permissibility of these procedures in religion, the necessity, and the most suitable breast surgery. I’m happy to address curiosity, providing insights from both a medical perspective (supported by science) and a religious standpoint (endorsed by fatwas obtained personally from authorities). My role as a board certified plastic surgeon in Malaysia extends beyond medicine, encompassing understanding on spiritual and personal levels, especially as a woman myself.

Hyaluronic acid yang medical grade, original, dan melepasi piawai (FDA Approved) tidak murah. 1cc sahaja seribu lebih. Anda buat filler 1L RM5000? Hampir semua kes saya, bila hantar tisu yang dah “Sebati” dengan filler ke makmal, mengatakan ia adalah menyerupai silikon / benda asing disamping parut dalam tisu yang teruk.

Day 0 - Pre Congress Live Surgery & Patient Representative Round Discussion.

Every time I see these neoclassical historic villas, I am reminded of all the Disney folklores, and my imagination starts to run wild with how people lived during those eras. I can never get bored with each unique classic villa that looks so regal and affluent, even though they are very dated. There's a certain charm that makes me fall in love with them every time I see one.

Fortunately, the Pre-Congress day is being held at one of these villas I admire so much—Villa Bergmann. It overlooks the beautiful view of Tiefen See lake and Babelsberg Park. This historic villa, built in 1891, once the residence of the renowned surgeon Ernst von Bergmann, has served as a center of education for over a century.

My day went from live surgery with open discussion to remote live video presentations and patient representative roundtable discussions, the day is filled with knowledge sharing about lipedema from both professional and patient perspectives.

The food (90% vegan) and hospitality are top-notch. So far, everything is going great!

I’ve decided to transform this old post into a reel as I revamp my IG layout. This before-and-after photo showcases one of my early mommy makeover transformations during my plastic surgery journey. Wanita Hari Ini: Demi Nak Cantik

I was recently invited to TV3’s ‘Wanita Hari Ini’ to discuss the pursuit of beauty - ‘Demi Nak Cantik.’ We talked about the darker side of the beauty industry, emphasizing the importance of public awareness, especially regarding invasive treatments.

As a plastic surgeon, we are one of the important key players, in the beauty field, particularly when it comes to invasive aesthetic or beauty treatments. I had the privilege of sharing insights alongside Prof Madya Dr. Nor Hayati Ibrahim, a senior lecturer in Clinical Psychology and Behavioral Health at UKM. During our discussion, we answered questions from hosts Fatin & Sugeeta, from discussing the risks that comes with any invasive beauty treatment, to delving into the psyche of unrealistic beauty expectation contributed by social media that is backed up by professional team and filters particularly.

I found it heartening that all of us share a common understanding that the desire to be beautiful is an inherent aspect of being human – a perfectly natural inclination. What becomes problematic, however, is when individuals take this desire to an extreme, prioritizing external beauty above all else, particularly when they establish beauty standards solely based on heavily edited references from social media, magazines and the big screens. It contributes to unwanted society stigma towards beauty treatment / plastic surgery, hurting those who truly benefits from it externally and internally.

This was my first appearance on live national television, and I hope we effectively conveyed valuable information for the audiences.

✨”Breast Filler Leads To High Infection Rate✨

In this photo is a patient when sought treatment at my clinic after developing complications from breast filler. As you may see, her main problems are her grossly deformed breast and back pain due to the size. This is her story before she came to see me.

She had received breast filler injections four times at her local beauty spa. Shortly after, she experienced multiple episodes of breast infection and abscess that required multiple surgical drainages. As a result, there are many scars scattered across multiple sites on her breast.

You might be wondering why there is a scar on her arm as well. The skin from her arm was used to cover the wound on her breast that couldn’t be closed due to poor wound healing.

Having such a diffuse foreign body (breast filler) greatly affects the blood flow and skin quality of the breast. This makes it difficult for infections to settle and for healing to occur. Additionally, she also had a history of breast implant surgery in one of her breasts to partly correct her devastating journey.

Meanwhile, breast sagginess progressed due to filler migration downward caused by gravity. Soon after, she finally came to see me to treat her symptoms. She underwent a two-stage surgery. Alhamdulillah, I’m so glad she is satisfied with her results. I could sense her mixed feelings of relief and regret when she hugged me during her follow-up.

However, I will never forget her response when I asked her, ‘Why did you do this to yourself?’ She said, ‘I didn’t know that hospitals offered beauty treatments. I thought they were only for sickness and diseases.’ Well, now she knows... and I hope YOU know too.”

✨Even Out The Uneven ✨

Pigmentation is a variation of normal skin tone. Uneven pigmentation sometimes can be a set back when wearing a lipgloss or liptint.

Appropriate laser of certain type can address these pigmentations. In this before after reel, shows quite a marked difference of lip pigmentation after one laser session, particularly on the left upper lip.

MDA approved laser machine that is only available in hospitals and eligible clinics is a very powerful machine. It needs to be done by LCP holder (either plastic surgeon, dermatologist or certified medical practitioner) to ensure things are done in the safest and most efficient manner.

✨Penyakit Untut @ Buletin FM ✨

Exactly a month ago, I was invited to give a talk about lymphedema, also called Penyakit Untut in Malay. It was for a radio talk show Santai Siang bersama DJ Marsila Hassan on Buletin.FM (they have recently changed their name to @kool101my .

Lymphedema / Sakit Untut is my most favourite reconstructive microsurgery and body contouring area of interest. I was excited to bring back to my country everything I learn during my fellowship in Austria.

So, to be able to share about this disease with the public was truly an exciting evening. Key points of it is to get early treatment using micro and super microsurgery technique as it can dramatically affect someones psychoemotional and socioeconomic impact for life. Plus, contrarily to public perception that it can only affects legs, it can also affect the arms, forearms and hand, especially in the case of post breast cancer removal. Infact, I also have had an experience with generalized lymphedema patient i.e lymphedema of the whole body!
